You can't "break" them of it. This problem will come and go for years and is a sign that they love you.
The biggest rule: NEVER sneak out. When you sneak out, you don't have to see her tears and panic when she discovers you are gone but they happen all the same. It also reinforces her belief that you are unreliable and will disappear any time she isn't in your arms.
When you leave her somewhere, tell her to have fun, that you love her, and that you'll be back soon. Then quickly leave.
When you come back, take a few minutes to reconnect. Ask her if she had fun. When she's older, ask her to show you something she played with. Let her know that she matters and you are glad to be back with her. Don't just grab her and race off to the car.
At home, when you leave her in the living room while you grab a glass of water, keep talking to her. Tell her what you are doing and that you will be right back.
