Cooked poultry -- 4 months
Uncooked poultry parts --9 months
Uncooked whole poultry --12 months
Cooked meat -- 2 to 3 months
Uncooked roasts -- 4 to 12 months
Uncooked steaks or chops -- 4 to 12 months
Uncooked ground meat -- 3 to 4 months

It's not so much that its past the freeze by date or will make you sick, but its at the amount of time its been frozen that will mean the quality of the meat is now garbage.
It also makes a difference if the ham is cooked already vs. raw. 1-2 months in the freezer for cooked, a year for uncooked.
