How can I stop my 2 year old from ripping her hair out?!?

I have a 2 year old who will be 3 in March. She has been consistently ripping her hair out for over a YEAR!!! We have tried a distraction toy on her clothes, she plays with it as she rips more hair out. We've tried hats, she just rips those off too. We don't know what to do to get her to stop but she is damn near bald on the left side of her head from the middle back, and its starting to spread to the right. Any help would be great. She does this when she's sleeping, when she's bored, and even when she's playing or eating. HELP!!

Have you tried just to keep it short, until she gets old enough to understand not to? That is all I can think of, sorry.
