Last rites is a collection of rites and sacraments (that's why it's plural). It usually consists of the Sacrament of Confession, the Sacrament of the Anointing of the Sick (which use to be known as extreme unction), and Holy Communion. When Holy Communion is giving as part of the Last Rites it's called 'Viaticum" which is Latin for "provision for a journey". Viaticum is seens as the food to strenghten a dying person for the journey from this world to life after death.
