What type of ID do you need for WIC?

I live in Ohio. What type of ID do you need to provide for eligible children? Can it be a SS card or do you have to have a birth certificate or do you need both?

  • In Tn you need a SS card and if they are an infant the card from the hospital, plus you will need to take them with you when you go.

  • Its alot easier picking up the phone and making a phone call than it is to wait around for answers. Every state has different rules.

  • In Toledo Ohio you need your ID and maybe a medicade card for the child...some proof of residency and income.
