Can i get shot record booklets elsewhere ? ( the ones that you get when they are born?)

When my kids were born I got little booklets to keep for their shot records.... during our move I have lost them and cannot find them anywhere....... their pedi. office said they would fill out another if we get one.... but I am not sure where I can get them... any ideas?

  • the drs office or hospital or a shot clinic. we have a clinic that offers free shots to all children under 19 regardless of income, and they have the booklet and they also forward the shot info to the school.

  • you can pick some up at the local hospital. Just give them a call.
