For RNs.....

I am in nursing school studying to be an RN. any tips on remembering everything to do in a brief assessment besides head to toe?

  • I start with introducing myself. Then I see if they are alert and oriented. Then I listen to heart, lung and bowel sounds. Then I check pupils and hand grasps. Then I check plantar flexion and dorsi flexion. Then I check their pedal pulses. Then I check anything specific, like dressings, incions or wounds. Check the skin every shift at least for breakdown. Look at the heels and sacrum especially. Do it in the same order as often as possible so you won't forget anything. Hope you have a great career as I have had.

  • Oh and I forgot radial pulses, I check those right before handgrasps.
