how do i go about putting the father on the birth certificate?

  • While at the hospital they will give you the birth certificate card to fill out. It has a place for the father's name on it.

  • your county records building should have some info on it. If not i'm sure you can call up the hospital and ask them. they probably get those kinds of questions all the time

  • While in the hospital, the father's name can be put on the b.c. if he is there to sign.

    If this does not happen, you have two options:
    1) ask him to voluntarily sign an acknowledgement of paternity affidavit (available at the child support agency). Then you can apply to have the b.c. updated.

    2) if he won't sign the acknowledgment, you can go to the child support agency and file a child support case. A judge will order a DNA test, and if he is the father, you will have a hearing regarding support, visitation, and updating the birth certificate.

    this is what i had to do because my fiance was in prison when our daughter was born in march and still is doesnt get out till december. takes a while but its worth it.

  • If you didn't get it taken care of in the hospital, you have to fill out an affidavit of paternity. In my state (Michigan) this can be done through the county clerk and the charge is $40 (At least thats what it was in 2006 when I did it)
