  • Not here in our state, you take it at the initial appt and then at your re-certification

  • Here is FL you only have to bring proof of income once every 6 months...and if they have medicaid or healthy kids you dont have to give income just there cards hopes this helps

  • if u are on title 19 ... (state insurance) that card should be enough...

  • Not in my state, just at the certification appointments. Just ask them when you go in when you need to bring it or not.
