when I was preg and using insulin .. I injected my long lasting (extended) insulin in my upper thigh .. and I injected my fast acting (rapid) insulin in my side below my hip bone and towards my back more...into a pinch of fat....

My stomach.  It's where I got told to do it.  Although it stinks when the baby kicks right as I do it!  Ouch!

I am not using insulin, but my sister injects her's into her arm.
