Sort of. DH and I knew the plan and made sure that our midwife knew that we weren't messing around. 

I'm a compulsive list maker, though, so planning made me feel more comfortable regardless.

I made one for my first and didn't for the next two.  We knew what we wanted so my husband was my advocate.

 I did have one, but it was pretty basic. Mine just said that I did want an epidural, a vaginal birth at all costs, and to breastfeed.

I printed 6 copies of it - with my name and information on it as well - for DH and the hosptial staff & shift changes so that everyone would be on the same page.

I got the birth experience I was hoping for :)

I didn't use one nor did I make one. Only thing I knew was that I wanted an epidural.
