Depends on the recipe. Most will give you some kind of idea of what they are looking for. I would always measure fresh, packed especially if cooking it as it wilts down so much.
Raw unless otherwise specified. And when raw, I pack it.

but as the others say usually it is uncooked unless it states cooked
