spelling curriculum, intent form issue. please help!

Instead of buying 'spelling' curriculum this year. I just went ahead and made up weekly lists of words he needed to know. Starting with all the high frequence words, and then adding in days of week, months of year, colors, time etc.
He practices them through out the week(reading, writing, reciting, games) and then takes a writen test at the end of the week.
How do I explain this on the intent form (a.k.a thorn in my side that must be turned in by Wednesday!)

I a still waiting to hear back from the district about whether I need an intent form or not. We are with an accredited program that is essentially a private school.  Good luck!

 Okay, I'll take a stab at it.  I would write:

We will be studying high frequency words seperated into lists of X number of words which he will practice over the course of the week culminating in a written test each (Friday?). 

Then I would probably attach one page with every word typed on it.

I know on the paper work I received the first time it said "author and title of book". But this is an e-book from these sites so I do not know if they will except it or not Hope it helps..

 make up your own booklet and give it a name and say your using _______ for spelling that year
