How would you word emergency medical forms for school?
Back history- exhusband is father of youngest 4 of my 5 kids but not my oldest who is 16 years old. Ex & I got together while I was still pregnant with my oldest (I had left his dad months before because he didn't want to be a father). Since he was able to talk my DS has called my ex "dad".
Ex & I divorced in April this year. I am now sitting here filling out emergency medical forms for school & it asks who the student lives with (I marked "mother"). It then asks for my contact info. Then it says:
"Father/Stepfather/Guardian/Foster Father (please circle one)" Then asks for contact info for this person.
How do I answer this question??? Ex has always been dad to my DS. He is no longer DS's stepfather (although we never actually put that on any of our paperwork before- we always said ex was his father on these papers in the past).
His bio father has never stepped foot in our state, not once tried to see his son, nothing but child support to acknowledge his "presence". He is not on DS's birth certificate (however neither is my ex).
Do I even write this info? Or put "no contact" & list my ex as an emergency contact? I want ex to be able to act on my behalf (or have the school contact him 1st if they can't get ahold of me for whatever reason).
