No but I live in Washington State and nobody does that here unless they are from the south.

 Yes. I only noticed it yesterday when my son said "Yes ma'am " to me. I guess it was second nature.

 I understand the Mr or Miss. However that is if they know the person. If it is a stranger like the cashier at walmart asking if they are enjoying their day. Would they say "Yes Miss" or "Yes Ma'am."

 My boys would answer with a simple yes or no.  They will sometimes add a thank you to it.  For example the cashier asks if they are enjoying their day and they would reply "Yes, thank you."
