I understand what the poster is trying to portray, but we don't have the whole story. Maybe the lady on the left is battling a disease? I think it's slightly unfair.

Very true!  I think they're trying to emphasize that staying active and healthy even in old age is very important.   My two late grandmothers were just like the two women in the picture:  one walked every day, at least a mile, and the other was more sedentary.  They lived to their nineties, and the active one was active and healthy looking until she had a heart attack and died.  The other one was practically crippled for the last few years of her life, and she was very thin and ill looking like the woman on the left.
