How do I fill out this application? It's very complicated!

It  asks if I have medicare what do I do? Check yes or no?  (i dont have medicare i'm only 23).  Then it asks for medicare number.  What do I put for that?  I receive medicaid and foodstamps do I need to disclose that information?  It's also asking me if I have pre-existing conditions.  Does PMS count?  thanks in advance.

Check no, and no do not disclose that info unless it is asked and gives a good vaild reason for asking. Also PMS doesn't count, PMDD does.

It does matter, lol.  If it were a job application, you wouldn't need to put that information, but with some other things, you would.  

If you have medicaid, say you have it and write down your number.  No, PMS is not a pre-existing condition.  It isn't a "condition" at all.

Yes, it does matter, but I'm stepping away now because it appears you have another pre-existing condition (rudea*s syndrome).

No to Medicare, nothing for Medicare number- Medicaid is different. Pms is not a preexisting condition.
