formula is fine for a 3 month old......yes. boiled water is too but not necessary.
If you're boiling to sterilize and then letting it cool, then yes it's fine.
Do not add boiling water to formula and give it to a child of ANY age.

You don't mix boiled water with formula. You put cold water with formula and then heat the bottle up, but don't heat it to boiling, or else you'll burn your baby's mouth. How can you have a 3 month old and not know this?
