Maybe you should feel ashamed for making snap judgements about someone you know nothing about.  It's very possible he was simply overweight, but then again, there could have been other issues.

Anyway, I look perfectly normal on the outside.  I'm not fat either.  But most would assume i'm lazy and stole a handicap card if they just looked at me.  I look fine BUT inside my body i'm far from fine.  I hurt all day long every single day!  Chronic pain sucks.  I'm still recouping from back surgery #1 and yes i've had idiots to say WHY ARE YOU PARKING THERE?  YOU SHOULD NOT PARK THERE.  I told one lady because I have back & leg pain and my dr prescribed the card to use.  She wanted to argue.  As if only 70+ yrs old can be handicapped.

It can be a hereditary disease. As a RN who works on the cardiac floor and over 75% of my pts are diabetic type 2 and obese and the doctors stress that we tell the importance of excercise and proper diet and weight loss, it does go along with obesity and several other factors such as diet and lifestyle choices, ethnicity, and genetics. Pts can go from diabetic to non-diabetic for type 2 if proper diet is followed and excercise and weight loss to the point of no longer needing medications for diabetes. It runs in my family as well and the family members who have it are obese. I dont have it yet. People have a different perception of what is obese these days as well. People consider a higher weight normal when it is really slightly obese or obese.

I'm not heavy either. Neither is my Mom or my Grandmother. They have it also. Well my Grandma had it. She passed away. My sister is overweight and she doesn't have it. I do. So I guess I would say that it varies.
