That would depend on the insurance policy you chose to purchase.

There is no such thing as "obamacare" each policy has things it MUST cover, but beyond that you can purchase any kind of plan you want.

 Huh? It covers what the insurance company decides it wants it to cover!  The government isn't issuing the plans-they are only mandating minimum levles of coverage (which they have been doing for decades now)

 The lw sets minimal levels of coverage. The insurance company vcan cover anything else it wants to cover. It can have a $500 co pay for Er visits or a $5 copay. It can cover invitro or not....

Yes and no. Your shorter statement implies that ALL of the coverage is dictated by law, and that is simply NOT true. Minimum coverages are established by law

Prescriptions drugs, well care, mammograms etc etc. An insurance company can cover things that are NOT mandated to be covered as well.
