If you mean in a restaurant, probably not, they will be happy in most cases to hold the cheese, but the extra meat or veggies they will probably charge for. Some may let you substitute one for the other, but most won't.
If you mean a calorie exchange, it means the total amount of calories of the cheese has to be known as well as the amount of calories added of either meat , vegetable or both.
