Was your cervic opened? It's not a black hole. Things can't just get lost in your vagina. You can forget it's there and jam another one in there. But it can't get lost.

It actually is possible, but not as in it's really lost. Your vagina is a pocket so it's in there but some people have "lost" one in there and couldn't find it or feel it. A couple things that can cause that is an overly full tampon being lodged to the side/back somewhere and probably feels like the rest of the "environment" in there. , or having a longer vaginal canal can cause one to get lodged further up where the woman has a hard time finding it.

I forgot a tampon before. So I guess you can say yes. I went to pee and felt something making its way down and was shocked it was a tampon. I made an OB appt and he said you'd be surprised how many get lodge in the vagina and some don't make there way down and are found after trips to the ER because the women are in pain
