You call another doctor to find out if they are taking patients and then you fill out their forms about transferring your records.

no you don't.  I switched drs and it was really difficult for me, so I feel you :-)  I didn't tell the doctor, just switched although my new caregiver was at the same clinic.

I don't think you have to, no. I think they get it when records are transferred. I never have anyway...

NOPE when I switched OBs and peds I just went to the new office filled forms out so they could request my records and moved on

You call up a new one and ask if they are accepting new patients, or you can get a referal from your old doctors office. I'd go through the front office staff for that.

It's easy. How did you choose this doctor? Same process.
