Diaper cake question

I looked up how to make a diaper cake and I got the general idea but I still have questions. The directions say use a bottle of lotion or baby wash as the center of the cake. How do you keep the cake from sliding off the cardboard base? The directions is for a 4 tier cake using a small baby bottle for the top two tiers. How do you keep the top tiers from falling off the bottom two? I was thinking hot glue but wasn't sure if that would work.

I make them. I use double sided tape. And ribbon. And if you do it right you should be able to put part of what you use as your center on your bottom two into the next layer.

i really dont know, my friend made me one, which was the first time she ever made one and she didn't "attach" it to the base. but to me it ok.
