How do you know when your baby is ready to wean??

They slowly cut back on how many times they nurse AFTER 12 months, they are not ready to wean before 12 months becuase it is nutritionally needed til then.  If they cut back nursings drastically before 12 months then theya re getting to many solids/liquids besides BM.

You know your baby is ready to wean when 

1- They are something over a year. (Or over two years if you don't want to have to give whole cow's milk)

2- They are eating a healthy varied diet. 

3- They lose interest in nursing and start nursing less frequently. If you just let them take the lead, they'll generally cut down on the number of feeds per day, then start skipping days, and then just stop one day.

Also good to keep in mind is that natural weaning can last a couple of days, weeks, months, or years. Weaning begins when babies begin transitioning to other sources of nourishment in addition to breastmilk.
