if babies diapers decrease.  you need to be changing at least 6 diapers(depending on babies age) in 24 hrs no matter if it's pee or poop.  That's the ONLY way to tell

Diaper count is the only indicator. 6+ for under 4mo then 4+ after that

Yep, if diapers start dropping off. If you see plenty of output than you know there is input. Same is true if dipes start disappearing.

Significant drop in diaper output, baby stops gaining weight or is losing weight.
