Medical Permission Form for babysitters/grandparents.

I'm trying to come up with a document that states that whenever my child is left with her grandparents and MINOR emergencies happen that they have the rights to get my DD to the dr for help while I'm unable to be reached and make decisions for her.

I'm reading online that this type of paper needs to be notarized, is that true? Can I just type something up in Word- print and sign it.

I know I need more than that..I'll be printing a copy of her shot record, our insurance cards and Rx cards along with medical history..etc.

Does anyone else have a document like this? If so, hows the wording?

  • It's a Loco Parentis (spelling may be off) and it has to be notarized. I have done them for my parents before so when I went away they could take my child to the ER in case of an emergency.

  • i would get it notorized to be on the safe side (most fedex places do it for 5 bucks). when did it for my dd who lived with my mom i gave her her health cards and ijust gave her a slip of paper saying that she is under the guardianship of so and so for an undefinite amount of time. my dd never had to go to the hospital but she took her to the dr for all of her newborn checkups until i moved in when she was 7 months old.

  • I have never had to have one notarized.  I write one for my in-laws when they go out of town with the kids.  My sister left her kids with my mom and dad and wrote a letter saying that my mom, me and my other sister were able to seek medical attention for them.  I ended up taking my niece to the doctor when I took my son, and they just made a copy of the note.  They didn't say that it had to be notarized.

  • Yes it needs to be notarized to be valid.  My husband did one for my mom.  He did it at JAG and they had a special form for it and notarized it there.

  • It actually depends on your state laws. .. In Ohio they don't need to be notarized. Your local hospital may have a blank form that you can use.. Our local hospital has one that is very similar to the one you sign for school.

    It is wonderful that you are taking these important steps now.... As an EMT, in Ohio, if Grandma is taking the kids to the park and is invloved in a fender bender without that form we can not let her take the kids...So make sure they keep a copy in the car too..

    This happens too often, and is spontaneous.

    Can I just do ONE doc per grandparent for them to keep for just in case something happens?

    Is there a expiration date on them?

  • I believe it does have to be notarized. I've signed a document for my parents like that. They are lawyers so they took care of all the legal paperwork, I just signed it. lol

    If you do the loco parentis you can make it for a few years. There is an expire date though.

  • You could put that it is valid until they are 18 or something like that. I think the one I signed said something along those lines.
