Can you help me on the 2nd page it tells you to list everyone in your house hold then on the bottom is starts asking for "Last Name of Child's Name" what child?it's confusing I have everyone listed on top Horizontal and then what? Is it asking for my dad's name wtf?? I don't get it. Too damn early in the morning I guess but I'm lost

you're filling it out.. you list you first, then your hubby, then the kids, then anyone else in the house
