This may get a tad long so I do apoligize.  my son was born 11 weeks early,,, along with his twin sister.  He is two years old now (spet 28th)  He has always bee quiet, although he did start to lagh at 6 months (before his sister) He is allergic to dust mites, corn, peanuts and soy.  (all mild allergies right now) the dust mites were wakig him up at night, we thought that is why he has a dark rig under his eyes.  so we bought the dust cover for the mattress and pillow.  It didnt help at all!  We are giving him his meds but its not doing anything.  he sleeps though the night now, however he still has the dark ring under his eyes and today i oticed it is reddish/purpleish.  My parents had me take him to the doc two weeks ago because something seems "off", he doesnt laugh as much anymore he hardly talks,, he maybe says 4 words.  he is mostly qiuet, he does have days where he will "talk" (babble) ut those are few.  The doc said he was fine and just going though his terrible twos, ut I am concerned that something may inndeed be wrong with him.  IDK, does anyone have any ideas?  I am very concerned.

Ask for a referral to a pediatric pulmonologist.  Start there.  Verify that it is just allergies and not asthma as well. (for info on asthma go to http://www.nationaljewish.org )  Two things jump to mind for me.  That he has asthma that isn't being treated so he is not getting proper rest and that he has chronic subclinical ear infections and so is not *hearing* correctly and is speech delayed.

I will look into the spec as far as hearing he can hear perfectly!  We get there hearint tested every three months.  He has only had one ear infection it was his twin that needed the tubes.  (she didnt walk until 22 months her doc didnt want to do anything until she turned two so I took her to the health department and they did the hearing test and she flat lined, one week later tues were in she was walking that day!)  I will ask for a referal, thank you.

I dont have any advice because i'm not a dr. and I dont have experience with the issues you are talking about. But I just wanted to tell you that you should keep looking for a dr. that will try and figure out what's going on. The fact that you have a gut feeling(or you wouldnt be posting this)...says that something may be wrong. Just get at least a 2nd opinion.

I have twins too and they do devlope differently.  However, you are the mom and you know your child best.  If you feel there is something going on, get another opinion immediately.  Make sure it is a professional who specializes in children.  It could be a spectrum of things.  Has he hit all of his developmental benchmarks?  Does he smile?  Does he make eye contact?  I would research a little on autism and see if any of that fits.  Also, seliac disease (I am probably spelling that wrong).  I am by no means a Dr. so I would trust your instinct and get a second opinion.

Good luck, mama.

Oh he makes eye contact ad he does smile just when you give him something to smile about (if that makes sense)

with subclinical ear infections they can hear and they have no symptoms of an ear infection.  The problem is that sinus swelling is encrouching on the estrauchion tubes and making them smaller, so the child is hearing like they are underwater.  They miss soft consenents for instance and their speech is delayed.   However when their hearing is tested it is fine... (went through this with my youngest ds).
