Photo printing permission Form?

I have taken several person's photos for senior portraits, family portraits etc and A few of them have been hasseled when taking their images to be processed and printed. I do this as a hobby not as a business but is there a form I should give them to take with their photos to be printed granting them permission to do so? What is this form called? I can't seem to get the search wording correct when looking for what I am.

 Yes, It doesn't have to be a "form" but a notice saying they have permission to print the pictures. Its just to protect the photographer from copyright infringement. Even if you're not doing it as a business they dont know that.

When I give my clients their CDs of pictures I have taken, I just make a little card that says, I _____ photographer  dba as -----------give _____ permission to print and distribute these pictures.  They have never needed it at a Target for instance but they have it just in case.  Funny thing, I have gone to Target, popped out the card and have had them hassle me on my OWN pictures!

I think you only need to get that form if you hold rights to your photographs
