Unintentional pregnancies?

For those of you who had unintentional or "accidental" pregnancies what were the circumstances (if you don't mind sharing)?

Condom Break? Pull out method failed? He didn't pull out in time? Didn't take birth control as prescribed?

I hear/see so many people saying they got pregnant unintentionally or accidentally and it just amazes me how often it happens while some people try and try to get pregnant and dont'/can't.

ETA- for those of you saying the withdrawal method failed can you be more specific? Did he pull out in time but you still got pregnant or did he just not pull out in time.

The reason I ask it that I've always used withdrawl method and just not had sex on days around ovulation. But somehow this week I miscounted or something. Thought I wasn't ovulating til this Monday or Tuesday but according to my chart I most likely ovulated the middle of this past week...

1 and 2 were planned 3rd, told by Dr's "can't have any more kids cos of PCOS, be grateful you have 2" (yeah really shitty Dr!) didn't use anything so very much a wanted surprise.

BC+Condom=FAIL with this one. I took my bc same day every day (with an alarm set) and it just didnt work. I guess it was ment to be

First baby I was 17 and got pregnant on the patch but had a miscarriage at 7 weeks along. Then I went on orthotricyclen lo and was on that for 2 years. Took it properly and everything same time everyday it worked until the month i got pregnant with my son. Don't know how or why it failed but it did. We used the pull out method after my son and then started trying for this baby in June 08 and didn't get preggo until february 09.

People never believe me but....we used a condom every time...never broke. I was on birth control and always took it on time, never missed a day. WE STILL GOT PREGNANT. One of GOD's little miracles and i couldn't be happier. My unplanned pregnancy turned out to be the biggest blessing in my life.
