Choosing Not to Vaccinate
I know this has been asked alot....
My son is getting ready to start daycare. I can't find the right website for vaccine exemptions. Please help me.
I live in Indiana.

by eykelley on Sep. 5, 2008 at 12:34 AMwhen my son enrolled in day care awhile back the daycare gave me the exemption form! So I would ask them, if they don't have one then I would go to the webpage the PP gave you!
