Forms/Letters for preschool to decline vaccines? Help!

Ok I have looked online at several different websites to see if I could find a form/letter to give to the preschool my son will be attending but I have come up empty handed. Can someone help me to derive a form/letter for declining due to philisophical reasons?

Also, I live in Ohio so I don't know if I need something specific.

Here you go. A brief look (and I may have not read thoroughly enough), shows some schools have forms, and there are links to forms by school district. Looks a little complicated, but it's all spelled out here for you.

Hola!

I am in Ohio as well. I just used something very basic for the philosophical exemption.

I am starting to worry, though, that she may not be able to go to the preschool for which we signed her up. I was talking to another non-vaccinating mother the other day, and she told me that privately-run preschools do not have to accept exemption forms. I did not know this. I thought the form was good for any facility.

I spoke with my daughter's teacher today about it, and she was pretty confident that the exemption form would be fine, but she encouraged me to speak with the director, just to make sure. God, I hope this will be okay. We've been talking to our daughter about going to school for MONTHS. She'll be crushed if she can't go. :(

Happy Day, Happy Day!

The director of the preschool called me back today, and said that all was well regarding my daughter's lack of vaccines. I told her I had the medical form from the school filled out and signed by the doctor, as well as the Ohio Exempt Form completed and notorized. She said "Oh, that's more than enough documentation." Horray!

School starts in two weeks. Wheeeeeeeeeeeeeeeeeeeeeeeeeeeeeeeeeeeeeeeeee!
