I use 2 methods, when I actually plan it out.
1. I make a Menu for the pay period (2 weeks) and make a list from that.
2. I take inventory of what we have, and adjust where needed. Most of the time I add stuff, but sometimes I have ingredients on hand.
3. I also try to keep certain things on hand: rice (brown, white, and wild), pastas, milk, eggs, certain canned veggies (green beans and corn), spaghetti sauce, Italian Dressing, Bread crumbs...things we tend to use alot. If I am running low on these things, they get added to the list.
I have a list pad on the fridge. Well 2 actually, one for grocery and the other for BJ's. I also have a 'meal plan' tablet on the fridge. Helps me compile the weekly grocery list. I found the meal planning pad at Michael's.

I am bad about a list, I usually buy the same things and just go thru the store.
