Failure to thrive

I took my son in for his 15 month well baby check up.  He was 22 lbs 3 oz (15th percentile) and 30.25 in long (19th percentile).  He has dropped in both percentiles.  He was 21 lbs 1 oz (23rd percentile) and 29.5 in long (40th percentile) back the end of aug for his 1 yr appt.  The dr wrote on my sheet to take home "failure to thrive"   Now i'm worried.  I knew he was on the smaller side but he looks fine to me.  Then i just looked up the info from his 1 yr appt and realized he barely gained any weight in 3.5 months.  She gave me a sheet on how to up his calorie intake and said i could get the carnation instant breakfast to add to his milk but i'm still worried. Seems liek such a serious diagnosis...Any ladies who have had this happen???
