Michigan.gov regarding all information concerning the MMMA including mock up forms that will be used for applications come April.
Remember: The state has 20 days in which to approve / deny your application and issue card - should you not receive a response or card within those 20 days you are deemed covered under the above mentioned MMMA. Please read more about it at the links above to educate yourself, your families, and your physicians.
