Cheated on this one for my niece. I used the domes from another cake. White canned frosting that I colored, and a new plastic dress up shoe that she later got to play with when I gave her the mate. I had made a mini chocolate wedding cake for her Daddy's wedding to her stepmom (by his request, if they bride was allowed to choose the big cake, he said he was allowed to choose a smalle one that looked like he wanted it to and put it beside the big one). My niece, age 4 at the time, said she wanted her own cake too, so I used what I cut from her Dad's to make one for her. She loved it.
