List one short-term and long-term goal that you feel is the most important to you. And why? Then list what you have to do to achieve these goals. Finally give yourself a time limit to have these goals completed. (Take your time to really think about your goals)

Short-term: Start the process for taking care of me and my child's mental health thru therapy.

Why? Because this effects the way we think and feel about ourselves and other situations. Mental Health treatment will help us and understand our conditions and how to cope and deal with them. How to not let these things control and steal the happiness out of our lives.

What I have to do: To start the process, request for us to be evaluated and then for treatment. Attend every session; if possible.

Time limit: To start this process, by July 8th, 2010

Long Term: To get a degree in the health care field either LPN/RN or Medical Assistant

Why? Because I really enjoy this field. I like to help others and reach out to people with compassion. I can have job security and make a decent income to support my family.

What I have to do: First, decide on one of them, complete all requirements for the program, enroll into the program, (keep grades up)study hard complete all assignments, complete program, graduate.

Time limit: January 2013
