I am filling out my "5 wishes" papers and I have a question...

where it says to put a person's name is there a way to put two people?

Like I want my husband AND my dad on it so they together have to make the decision?

IF I do fill this out and put both would it still be leagle?

Do you cut or have you cut PLEASE come and talk things out with us we're a new group and its ONLY for cutters or self hurting issues!! nothing leaves our group its all in privacy!!

You can put both but you must place one as primary and one as secondary. Otherwise there will be a legal battle if they disagree. The secondary is only if the primary were, say in an accident with you and unavailable. I would speak to them and let them know you trust them to communicate and you want them to jointly decide but you must choose one as primary. Hope to help.

Its like a will.. its from exempla.its

1. the person I want to make care decsions for me when I can't

2. The kind of medical treatment I want or don't want

3. How comfortable I want to be

4. How I want people to treat me

5. What i want my loved ones to know
