Diapers, wipes, clothes (including socks), extra snacks, hair ties, some small toys, thermometer, meds, tissues. Oh, and this was for my DD who just turned 3. We don't carry it anymore now that she's potty trained. When she was younger we would of course keep bottles, formula,water, and other baby type things. Lol.
DS is 2 now so usually Diapers, Wipes, a snack, a couple bottles of water, a clean sippy cup, and a few small toys. When he was a baby I always carried bottled water, formula, diapers, wipes, diaper rash cream, extra clothes and bibs, burp cloths, toys, clean bottles/sippy cups, his lovie, and extra paci's until he quit taking them at around 7 months

Diapers, wipes, desitin, lotion, sunblock, sunglasses, baby clothes, toddler clothes, snacks, toys, crayons, meds, water, wallet, small notepads, cupons, a calendar, pens, straws, napkins, breast pads, feminine pads, camera, phone charger, camera cord, whatever's in my hand when I'm getting ready to go... Um...
My son is 3 1/2 months so right now I carry: burp cloth, changing pad, blanket, diapers, wipes, formula, vaseline, pacifier, extra onesie, socks and toys.
