If the over the counter's don't work, our nurse practicioner gave me an awesome remedy when ds had a bad diaper rash caused by medicine.
Get a take and toss plastic bowl with a lid and mi1 large tube of desitn (purple tube) 1 large tube of lotramin AF and 1/2 large container of vaseline. Mix it together and apply it to the affected area. I keep the leftovers under the sink for future use. It cleared him up in a couple days. He was 6 months old when we did this, so if your son is younger, I'm not sure I'd use it without doc's permission.
Something else you can do to help him. Instead of using a wipe or wash cloth, use cotton balls and mineral oil to clean his bottom after a dirty diaper. The alcohol in the wipes irritate diaper rash.
