This is my revised birth plan after I posted my last one online for you guys to review for me. I took some of your suggestions and re-wrote it to be shorter and less wordy. any one have any input or anything I may be missing?

Due date: 9/8/2011

Patient of

Scheduled to Deliver at:

I know that things change, but unless there is a genuine medical need I would like to stick to this plan as closely as possible.

Sincerely,

Stephanie

Labor/delivery coaches: Husband- and Doula . All other persons should be directed to the waiting room.

Labor/vaginal delivery

-         free to move, walk, and change positions throughout labor including birthing ball and shower use

-         fluids and light foods by mouth throughout labor

-         a peaceful environment- lights low and quiet throughout labor and delivery

-         vaginal exams to a minimum

-         no IV

-         wear my contact lenses or glasses throughout labor/delivery

-         no continuous/internal monitoring

-         no artificial rupture of membranes or pitocin

-         I do not want to be offered any pain medications. If I want something I will ask.

-         Should I choose to use a pain medication I would prefer a walking epidural

-         Prefer to tear to an episiotomy

-         Prefer perineal massage to avoid tearing

-         Should I tear I would like a local for the repairs

-         Allowed to choose the position I deliver in

-         A mirror to view baby's head crowning

-         Wait until I feel the urge to push before beginning

-         Baby placed directly on my chest immediately after delivery

-         To "catch" the baby

-         Allow the cord to stop pulsing prior to husband cutting the cord.

-         Hold baby during delivery of placenta, nurse immediately, and all initial exams performed on my chest.

-         All exams/bathing in my presence. IF she must go to the nursery for examination Daddy should accompany her.

-         See the placenta after it is delivered.

In Case of Cesarean:

Unless absolutely necessary, I would like to avoid a Cesarean

-         If my primary care provider determines that a Cesarean is necessary I would like a 2nd opinion if time allows

-         To be fully informed and involved in decision making

-         husband present at all times

-         An epidural

-         If she is not in distress she should be given immediately to husband

-         Sam should always be accompanied by husband

-         Nurse as soon as I am in recovery

Postpartum:

-         A private room

-         I do not wish to be separated from my baby at any time

-         I plan to breastfeed exclusively. No Bottles or Pacifiers should be offered

-         To meet with a lactation consultant

-         My son to visit us in the hospital

Sounds good and gets all the points across! Not to long to just skip anything over. Good job!

Looks much better! I would add not to cut the cord until it is done pulsating even with a c-section.
