In the wise words of my doctor, when its the real thing, you'll definitely know it for sure.
For me, BH was just my belly getting tight (especially at the top) and not was sort of uncomfortable and not painful.
The real contractions I felt starting in my back and wrapped around to my lower belly. Super painful and I could definitely tell the difference when the time came.
My whole pregnancy I figured, if I'm not sure then it's probably not the real thing. And I was right :P
Timing also really helps.

Mine were like terrible cramps that started in the front lower belly area and radiated to the back

When I have contractions, they seem to be really low and so painful I can't speak during them.

They're different for everyone. Most people feel them in the abdoment, some only feel them in the back and I have a friend that went into labor with her first 3 without knowning because all she was getting was cramps in her thighs.
