Diapers, wipes, extra clothes, some kind of snack, cup, water, and a few toys :)
well the list is short now that my dd is potty trained. it also depends on where we are going and how long we will be gone. usually: change of underwear (or 2) for both, extra pants for DD, wipes and snacks/water. if it's for the park, add sunscreen. otherwise, i don't bring much.
After they were potty trained, in which case I just brought an extra diaper in my purse, we never used a toddler bag or anything like that for just normal going out. If we are just going out for a little while to the grocery store or something, they dont need toys and snacks, too much to carry around.

Water bottle
For Nik we have , extra clothes, several pair of undies, a drink, cars, something to color
For Anna, diapers, wipes, drink, clothes, shoes(she always takes them off), tylenol, and toys.
