Provider

My name is Leslie Simmons. I have 5 children. I have been caring for children in my home for 15 years. There may be up to 5 pre-school age children in childcare at one time. My goal is to provide a respectful, safe environment for children to grow and learn. Daily activities include creative periods, constructive play, reading, and quiet time. Two nutritious meals and 2 snacks will be provided. A dog and a cat share our home.  Also, we have a creek that runs through our yard. Children will be in a fenced play area while we are out of doors. I take the children outside every day, weather permitting.
This a registered childcare facility, which means I don’t make the rules; New York State does, but I must enforce them.

Discipline


I use positive reinforcement and if required attempt redirection. If needed the child may be removed from the situation for a brief time out, not far from the others, where the problem is discussed with the child.

Meals and Snacks


I will provide breakfast, morning snack, lunch, and afternoon snack. I emphasize nutritious foods such as whole grains, fresh fruits, and vegetables. Milk or water will be available to drink. Lunch will include meat. If your child has special dietary needs, please let me know so that we can accommodate them.
If you would like to send a treat for a special occasion such as a birthday, please let me know ahead of time and be sure to bring enough for everyone.

Clothing and Supplies


Please send at least one full change of clothing in case of accidents. We all have accidents from time to time. We will be going outside each day. Be sure your child has snow pants, hats, and mittens in the winter months and in the summer please send sun screen. Parents are responsible for providing diapers unless other arrangements have been made.



Sickness


This is a well and mildly ill childcare facility. Exclusion criteria is as follows:
vomiting
diarrhea
fever of 101 or higher
mouth sores/impetigo
conjunctivitis (pink eye)
head lice
scabies
any unexplained rash
any contagious illness
If your child is ill, please do not send them. If they become ill and you are notified, please pick them up promptly. If possible designate an alternate person to pick up your child incase you are unable to do so.
I can not give any medication. If a child requires medication during child care hours you or another relative must come and administer the medication to the child.
Should a child become seriously ill or injured during care and require medical attention, I will notify parents as soon as possible. In life threatening situations, I will first call 911, then notify the parents. I must have a signed medical authorization form on file in case parents can not be reached.
If my children are ill or if I am mildly ill, but still able to provide care, I will notify parents as soon as possible and it will be your choice weather or not to bring your children.

Payment


There is a $5. registration fee due at the time of enrollment. I also require a deposit equal to one weeks care ($100) This may be divided in up to 5 payments due the first 5 weeks your child is in my care and used for your last week of care.
I charge a rate of $100. per week to be paid weekly due on Friday of the week prior to care. Payment is due weekly whether or not your child is in attendance. If your payment is over 1 week late, your child will not be able to return until it has been paid.
Tax statements will be handed out at the end of the year.
All returned checks are subject to a $25.00 fee. If this happens twice, all further payments will need to be made in cash.


Another option is to pay $25 for each day or $10 per part day (less then 4 hours) that your child is in care.  Payment may be made on Friday of the week that care is provided.   If your payment is over 1 week late, your child will not be able to return until it has been paid.

Before/after school care will be $45. per week or $10. per day.


Hours of Operation


I will be open from 7:00a.m. to 5:00p.m. Monday through Friday.
You may pick up or drop off your child any time during these hours. If you require care before 7am or after 5pm, you must make arrangements ahead of time and there will be an added charge of $3. per hour. If you are over 10 minutes late to pick up or early to drop off your child without prior arrangements being made, you will be charged $1 per 5 minutes.

Vacation and Holidays


I take 2 weeks paid vacation per year and observe all major paid holidays. A calendar for the coming year will be provided by December 15 of the prior year. If you decide to take a vacation, you must still pay for childcare since I will be holding your child’s spot. Please notify me in advance of planned absences so that I can plan activities accordingly. If your child is not in attendance for more than 1 week, without any word from you, at the beginning of the second week, I will consider his or her spot open.

Confidentialit
y


Personal information about children in my care and their families is strictly confidential. Please do not ask me to reveal information about children other than my own. A health history and other family information will help me provide the best care for your child. However you do not need to reveal any information you do not wish to, except information required by state law.
As a mandated reporter, I am obligated to report any cases of suspected child abuse.

Infants


I can legally have 1or 2 infants (under 24 months) in my care. At no time will there be more than 2 children under the age of 2 in childcare. My decision on whether to take a second infant must be based on whether I feel that I can adequately care for both infants while also conducting activities with the toddler/pre-school group.
Infants will be in the same room as older children. " Pack and Plays" may be used for napping. Infants may be placed in a swing, bouncy seat, or exersaucer or carried in a sling. "Baby Signs" will be used to enhance communication.
Cloth diapers will be provided while the child is in my care. Please send one diaper for your child to wear home or simply return the cloth diaper that he or she is wearing the next morning. (please dispose of any solid waste, rinse in cold water and place in a plastic bag).
You must supply your own breastmilk or replacement formula and bottles.

wow i wish u were closer lol ive been looking into daycares and most around here have bad reviews and my daughter is 4 months so top revieews are needed

Well I am no longer registered.  I haven't had any kids in a while and it was just too much to keep up and not receive anything in return.  I will accept children tho, if they come along at $20 per day.  Iwill keep the same schedule and my home is still up to codes with the exception of the door alarm and pad locks on the gates (they just seem a bit ridiculous) I don't think the kids will want to escape that badly! And I can legally only care for 2 children of any age on a regular basis with out reporting to the county.
