ive known people whos 4 yr old has a skateboard. however i have told everyone i know not to give one to my kids because my 19 yr old brother decided to get drunk and be pulled by a rope attached to a truck while on his skateboard and busted his knee cap in 3 places. so make sure ur grandsons parents want him to have one before buying one

My son got one at about that age.  He always wanted me with him, and I was happy to go to be sure he was safe.  I think it's a reasonable request.

My son is a 21 yr old street-skater.  He has injured his knee a few times, and broke his arm once.  He also has plenty of road rash.

All of these injuries can happen doing a variety of other sports.  Some ground rules while they are young and impressionable are a must.

Protective gear, no skating in the street, supervision by someone responsible.  Is there an indoor skatepark in your area where they give lessons?  Good luck!
