GO TO SOMEONE HOMEPAGE AND UNDER THERE PROFILE PIC, THERE IS A BUTTON THERE THAT SAYS FRIEND REQUEST, CLICK ON THAT AND LEAVE THEM A MESSAGE.

Go to the page of the person you would like to send a friends request to, on their home page, below their picture, will be the option to send a friend request. Click that on and it will take you to your inbox, then you can send a friends request and have the option to send along a little note. :)
